Patient Navigator Non-Clinical 2
Salary Range: $17.77 - $35.73 Hourly
Final Filing Date: 07/28/2022

The role of this position is to provide executive level input to ensure that families are equal partners in the planning, development and implementation of services. Using a trauma-informed approach, the Family Navigator assists families with accessing needed community resources and navigating county systems, providing education related to treatment and resources, coordinating with patient’s community service providers. Identifying patient’s preferences and any barriers to the treatment plan implementation.

• Position requires requires the employee to have reliable transportation

• This position may be subject to a criminal background investigation, drug screen, Live Scan fingerprinting, medical evaluation clearance, and functional capacity assessment.

Required Qualifications
• Applicants are encouraged to upload license and certification if required of the position.
• Valid CA Drivers License
• Current or previous experience as a family member of a child receiving public mental health services or housing services
• Excellent interpersonal and communication skills: verbal and listening. Ability to work diplomatically with a variety of people. Ability to convey instructions, directions, and information appropriately and accurately. Ability to work as a team member.
• Ability to communicate and interact effectively with children and their caregivers, other staff members, social workers, and representatives of community agencies.
• Ability to work independently with a minimum of directions. Ability to exercise discretion, independent judgment, with decision being reviewed if they represent departure from departmental goals.
• Ability to organize a variety of concurrent activities, set realistic goals and objectives, plan ahead and meet deadlines without compromising quality.
• Ability to direct the action of others effectively in order to accomplish tasks and functions.
• Good writing, verbal, and planning skills. Ability to write clear, concise notes using applicable computer software.
• Strength-based perspective that is culturally sensitive.
• Ability to work collaboratively with other professionals.
• Good decision-making skills and use of sound judgment.
• Ability to successfully manage conflict.
• Knowledge of child, elder abuse, and domestic violence reporting requirements, and other significant legislation and laws regulating mental health practice.

• Ability to use supervision to enhance professional growth and examine performance outcome
• Skills, knowledge and ability to support the policies and procedures of the CAARE Center and UC Davis Health.
